WAIVER: Please accept my school to participate in the FGMC to take place from March 20-May 10, 2017 as part of the Fifth Third River Bank Run and the Fifth Third Junior event. I represent and warrant that I am authorized to contract on behalf of my school and/or district, and waive any rights I or the school/district may have against Fifth Third Bank (including, its parent, subsidiaries and affiliates), its employees, directors, officers and agents, including but not limited to USATF officials, volunteers, consultants, or sponsors for any fatality, injuries and/or damages, including, but not limited to, demands or actions for negligence, premises liability, emotional injury, intentional conduct, tort claims, and any other actions or demands of whatsoever nature I have or may have in connection with my participation in the Fifth Third River Bank Run, and otherwise agree to hold these entities harmless for damage or injuries occasioned by my participation in any Fifth Third River Bank Run event. I also authorize Fifth Third Bank to utilize any photographs, personal narrative, interviews, or audio and video recording of my school's participation in any Fifth Third River Bank Run event for any and all purposes.
AGREEMENT: By signing this Electronic Signature Acknowledgment Form, I agree that my electronic signature is the legally binding equivalent to my handwritten signature. Whenever I execute an electronic signature, it has the same validity and meaning as my handwritten signature. I will not, at any time in the future, repudiate the meaning of my electronic signature or claim that my electronic signature is not legally binding. By signing below, I accept the conditions of this agreement.
